You may schedule an appointment by calling our office during regular business hours.
Our office staff is available from 9 a.m. to 5:00 p.m. Monday through Thursday and
9 a.m. to 1 p.m. on Fridays. Both morning and afternoon hours are available. We
will always try to accommodate your needs by time, location and physician preference.
For your first appointment, please arrive 15-20 minutes early to fill out necessary
paper work. Also, please bring the following:
- Relevant medical records
- List of medications you are currently taking
- Your insurance card
- A referral if necessary
If you cannont keep an appointment, please provide us with 24-hour advance notice
of your cancellation.
